Problem Solving Exercise LWS101, Semester 2 2016
Evie is a registered nurse who works in the oncology ward at a regional hospital in Queensland. One of her patients, Ragu, is suffering from bowel cancer. Ragu?s prognosis is very poor and his life expectancy is short. In fact, the treating team does not expect he will live for much longer. Ragu?s condition has deteriorated in the last 24 hours and his pain has become significantly worse. Ragu?s family have been in constant attendance. After spending some time speaking to Ragu and his family, Dr Fox gives Evie a written medication order for a higher dose of pain killer. The doctor says to Evie, ?Ragu is in a lot of pain. Administer this medication in accordance with the script.? Dr Fox then departs to attend to other patients.
Evie administers the dose of pain killer in accordance with Dr Fox?s written order but Ragu?s condition does not improve, in fact it worsens. Ragu begs Evie to increase his dose as his pain is unbearable. Evie can see Ragu is in a lot of pain but she is concerned that an increase in dosage might put Ragu at risk of respiratory depression. Ragu continues pleading with Evie to increase his dose and put him out of his misery. He states ?I?ve had enough of this life ? Please just let me go in peace.? Evie rings Dr Fox who advises Evie verbally over the telephone to administer a higher dose of pain killer.

Apply the ethical and legal decision-making framework to this scenario to advise Evie about how she should proceed.
